Medical Disclaimer
Information on the Site is provided for general educational and informational purposes only. It is not medical advice, diagnosis, or treatment, and it is not a substitute for consultation with a qualified healthcare provider. Use of the Site does not create a provider-patient relationship. A provider-patient relationship is formed only through an established course of care with the clinic.
No Emergency Services
The Site and any associated communication methods are not monitored continuously and are not intended for medical emergencies. If you are experiencing a medical emergency, call 911 or seek immediate in-person care. Do not use Site forms or email to communicate urgent or emergency medical needs.
Appointment, Payment, and Membership Policies
Fees, appointment policies, and membership terms are communicated before services are rendered. Payment is due at the time of service unless otherwise agreed in writing. Membership services are non-insurance-based and are designed to support ongoing functional medicine care. Membership terms, inclusions, and pricing may change with appropriate notice. Membership is not health insurance and does not satisfy any individual health insurance requirement.
Acceptable Use
You agree not to use the Site to: violate any law; infringe the rights of others; transmit harmful code; attempt to gain unauthorized access to any system; scrape or harvest data; or interfere with the operation of the Site.
Intellectual Property
All content on the Site, including text, branding, logos, graphics, and written materials, is owned by or licensed to Lumae Functional Health and is protected by applicable law. We may revise these Terms at any time by posting an updated version on the Site with a new effective date. Your continued use of the Site after changes are posted constitutes acceptance of the revised Terms.
Severability and Entire Agreement
If any provision of these Terms is found unenforceable, the remaining provisions remain in full effect. These Terms, together with the Privacy Policy and the HIPAA Notice of Privacy Practices, constitute the entire agreement between you and Lumae Functional Health regarding the Site.
